OUR HEALTH PARTNERSHIP ARE LOOKING FOR CARE CO-ORDINATORS 

 

We are seeking Care Coordinators to join our growing team at Our Health Partnership, working with our 8 Birmingham OHP supported Primary Care Networks (PCNs).

Each person will be based in one of the PCNs, listed below. The bases are to be confirmed.

OHP Supported PCNs:

Alliance of Sutton – Sutton Coldfield
Moseley, Billesley and Yardley Wood – Yardley Wood
Kingstanding, Erdington and Nechells – Kingstanding
Bournville and Northfield – Northfield
Harborne and Quinton – Harborne
Weoley Castle and Rubery – Rubery
Shards End and Kitts Green – Shard End
Bordesley East – East Birmingham Area

The post holder will work for and across the Birmingham Primary Care Networks, however will be primarily working for one PCN.

The successful candidate will be part of a dynamic and forward-thinking PCN led Multi-Disciplinary Team (MDT). Leadership will be provided by the PCN Clinical Director and Consultant Pharmacist at OHP.

We are looking for individuals who will coordinate the work of healthcare professionals and non-clinical staff including volunteers involved in the care of patients registered at GP practices within the wider PCN population.
A key part of the role of a care coordinator role is in the Care Homes MDT.

The successful candidate will act as a point of contact for residents, families and professionals who visit care homes, such as MDT members and in-reach specialists. They will support the MDT with the weekly virtual home round through identification of people in need of review, or collation of information on people requiring an MDT review in addition to providing coordination, secretarial and administrative support to the MDTs within a single or multiple PCNs.
