Our Health Partnership is passionate about how we get the best for our patients, and in our view getting the best for our patients means, not just talking about joining up care, but actually doing it. We believe our patients should have more control over their health and their treatments. To achieve all of that we know that patients must be supported in their communities.
And that kind of support will rely on high quality, highly motivated GPs, working with innovative partners, able to deliver, holistic, personalised, continuity of care, direct to our patients, on their doorstep, through the NHS and the public and voluntary sector working together.
As a GP partnership with 46 surgeries, covering Birmingham, Wolverhampton, Solihull and Shropshire, we are exploring how we can work with everyone involved in health and social care. We aim to keep our communities healthy, to keep patients out of hospital and to deliver first class healthcare closer to people’s homes.
Our GP Board members are actively involved in the development and implementation of the Service Transformational Plans (STPs) influencing the way services should be linking together. We are actively working with our partners and working through plans we can connect Our Health Partnership with health and social care professionals in hospitals, local councils, national charities and with Birmingham’s and Shropshire’s vibrant voluntary sector to find innovative solutions and practical answers to our diverse challenges.
